We participate in the Ophthalmology Fellowship Match for Ophthalmic Genetics. All applicants must register with the Ophthalmology Fellowship Match (SF Match) and obtain a registration number before applying.
Applications are accepted only through the Central Application Service (CAS) on the SF Match website.

2. View Our Program Listing
After registering, view our listing in the SF Match directory for detailed program information and requirements.
3. Submit Required Materials via CAS
Your CAS application must include:
- Completed online CAS application form
- Three letters of recommendation
- CAS distribution list and payment
Criteria for Applicants
Minimum Requirements
- Completion of an accredited ophthalmology residency program.
- Eligibility for medical licensure in the state of Iowa.
- Foreign medical graduates must:
- Hold a current, valid ECFMG certificate or
- Have completed at least one year in an accredited U.S. internship or residency program.
Selection Considerations
Applicants are evaluated based on the following:
- Ophthalmic Knowledge Assessment Program (OKAP) scores
- U.S. Medical Licensing Examination (USMLE) scores
- Letters of recommendation
- Personal statement
- CAS application materials
Interview Process
- Interviews are by invitation only and arranged through the Fellowship Director’s office.
- Only candidates under serious consideration for appointment will be invited to interview.
Licensure Requirement
All fellows must obtain Iowa medical licensure prior to the start of the fellowship.
Deadlines
- Application deadline: September 15 of the year prior to the July start date
- Interviews begin: October
- Iowa license deadline: June 30
- Fellowship duration: July 1 to June 30
- Orientation start date: July 1
